- Performs all required responsibilities/duties addressing the ages and condition of the patients served.
- Responsible for direct patient care which is determined, observed, supervised and documented on plan of care by a professional clinical provider.
- Performs delegated tasks as authorized by the professional clinical provider depending upon situation and HHA experience/competence.
- Performs routine household tasks as related for a safe, healthy and comfortable environment for the patient, as instructed and documented on plan of care by the professional clinical provider.
- Documents patient status and care given, as reflected on required records.
- Works as part of the Home Healthcare team participating in and identifying ways to improve the quality of care being provided to the patient.
- Demonstrates respect and regard for the dignity of all patients, families, visitors and fellow employees to insure a professional, responsible and courteous environment.
- Promotes effective working relations and works effectively as part of a department/program team inter and intra departmentally to facilitate the department's/program's ability to meet its goals and objectives.
- Attends all required safety training programs and can describe his/her responsibilities related to general safety, department/service safety, specific job-related hazards.
- Follows the home and post-acute services division's exposure control plans/bloodborne and airborne pathogens.
- Complete all educational requirements as assigned.
- Required High School Diploma or equivalent
- 1 year of relevant experience 1 year of nurse aide or equivalent Preferred
- CRT-Nursing Assistant - PROMETRIC Prometric Upon Hire required
- CRT-Basic Life Support (BLS) - AHA American Heart Association Upon Hire required
