Job Summary
Spectrum Health is developing a multidisciplinary practice to provide the full spectrum of mental health care to meet the needs of adolescents and young adults. Serving patients ages roughly 16-26 years old, this team will include medical providers, psychotherapists and psychologists.
- Performs diagnostic examinations of the patient's psychosocial and social functioning and adjustments to their medical and/or psychiatric disability/illness.
- Collaborates with the patient and members of the multidisciplinary team to develop treatment plans and goals consistent with the needs of the patient.
- Performs individual and group treatment interventions in a manner, which meets the standards of the profession and changing needs of the patient.
- Documents plans, interventions and patient's response/involvement according to standards.
- Identifies and documents discharge plans which meet the needs of the patient and are consistent with resources available.
- Participates in ongoing quality monitoring, policy review, and process improvement activities.
- Maintains updated knowledge of rules, regulations, and standards in the field of social work, behavioral health and related matters of interest.
- Ensures that patients, their families, visitors, customers, and employees are treated with kindness and respect. Demonstrates teamwork and integrity in all work-related activities. Ensures that work-performed supports Spectrum Health's mission of Superior Quality Care.
