Job Description

Job Summary
  • Collaborative, multidisciplinary team inclusive of psychiatrists, psychiatry APPs, psychologists, LMSW psychotherapist
  • Hybrid model of on-site visits and virtual
  • Professional development opportunities through multidisciplinary case reviews and integrated care rounds
  • Annual Psychiatry & Behavioral Medicine symposium offering desired topics such as ethics, human trafficking and pain
  • One electronic medical record system allowing easy communication and coordination among all care providers

Provides diagnostic and psychotherapeutic interventions for patients of all ages within the Spectrum Health System.

Essential Functions

  • Performs diagnostic examinations of the patient's psychosocial and social functioning and adjustments to their medical and/or psychiatric disability/illness.
  • Collaborates with the patient and members of the multidisciplinary team to develop treatment plans and goals consistent with the needs of the patient.
  • Performs individual and group treatment interventions in a manner, which meets the standards of the profession and changing needs of the patient.
  • Documents plans, interventions and patient's response/involvement according to standards.
  • Identifies and documents discharge plans which meet the needs of the patient and are consistent with resources available.
  • Participates in ongoing quality monitoring, policy review, and process improvement activities.
  • Maintains updated knowledge of rules, regulations, and standards in the field of social work, behavioral health and related matters of interest.
  • Ensures that patients, their families, visitors, customers, and employees are treated with kindness and respect. Demonstrates teamwork and integrity in all work-related activities. Ensures that work-performed supports Spectrum Health's mission of Superior Quality Care.

Qualifications
  • Required Master's Degree Social Work
  • LMSW License
